Connect Well is a new social prescribing service, which works in partnership with primary care to navigate people to relevant services and support groups within the community.

Aims of the service

Our aim is to connect people to services and activities in their community in order to benefit overall health and wellbeing.

We believe that everyone can live a fulfilled life, however, sometimes people need help to achieve this.

Feeling connected to your community has lots of health and wellbeing benefits that support resilience, independence and good physical and mental health.

Our Wellbeing coordinator is Andrea Vickers. Andrea offers appointments every Wednesday morning at Wetherby Surgery and alternative Fridays at Harewood Surgery. For more information, please ask our reception team.

Health Coach Clinics

For any patient who are struggling to lose weight, have high cholesterol, high blood pressure, pre diabetic, high BMI, wants lifestyle advice or just wanting to get back in to exercise.

Our Health Coach Nicola is a trained personal trainer and healthcare professional who specialises in lifestyle medicine.

Nicola offers:

  • Health and wellbeing advice and support
  • Diabetes management
  • Weight management
  • High blood pressure management
  • Anxiety and stress
  • Exercise classes
  • And much more!

New Service Provision

New service provision starting 9th September

We are excited to announce that we have 2 healthcare professionals joining the team in September to compliment the services we currently offer. 

2 very experienced Paramedics that have spent many years working in Primary Care / General Practice will be supporting the delivery of urgent on the day appointments and the home visiting service. This service has been trialled and tested across the country and has proven to be very effective; both in terms of supporting patient access to services and positive patient experience. This model of care has also been shown to enable patients to access the professional best suited to their needs quicker. We have therefore made the decision to implement this as a 3 month trial within our service.

They will be joining our team on the 9th September and will be working very closely with our existing team of GPs, Nurses and other Healthcare Professionals to provide urgent, on the day assessment and treatments.

What does this mean for you?

You will still call and book an appointment in the same way. For patients that need an urgent telephone or face to face consultation; and for those that need a home visit,  in addition to our current team members, your needs may be managed by our highly skilled Paramedics. You can expect to receive the same level of care that you currently receive.

Did Not Attend Stats Update

Please note we had 52 appointments where the patient did not attend in January 2019 which resulted in a lost of 780 minutes of clinical time.

If you are unable to attend your appointment please let us know as far in advance as possible. This ensures we are able to see as many patients as we can.

Evening appointments

There are now extra routine appointments available in the evening, making it easier for you to get an appointment at a time to suit you.

The practice offers:

A virtual clinical pharmacist service

This is a telephone appointment for patients who: need to speak to a pharmacist regarding a medication issue; but don’t need to come to the surgery. The pharmacists are able to prescribe and organise prescriptions electronically. 

A virtual musculoskeletal (MSK) service

This is a telephone appointment for patients who need to speak to an advanced physiotherapist regarding muscle and joint problems. They will issue advice and where appropriate arrange a follow up.

A virtual mental health support service (Healthy Minds)

This is a telephone appointment for patients aged 18 and over who would like help with emotional wellbeing. It will provide a 20minute telephone consultation with Healthy Minds Practitioners who will be able to offer support for patients experiencing low mood, stress or anxiety. They will offer support, explore and develop coping strategies and offer signposting and recommend other support services. Saturday and Sunday morning face to face appointments

There are now Saturday morning routine appointments from 9-12pm available through our extended access service at:

Wetherby Health Centre, Hallfield Lane, Wetherby, LS22 6JS

This increased availability is an extension of the services offered by your practice and is provided by a range of local clinicians including GPs, nurses and health care assistants.

Please note, this is not a walk-in service – you still need to make an appointment via your own GP practice. Extended Access appointments are for routine general practice issues and not for urgent care. If your practice is closed and you are unwell please call NHS 111.

There are now routine appointments on Sunday morning from 9am-12pm available through our extended access service at:

Rutland Lodge Medical Practice, Scott Hall Road, Leeds, LS7 3DR

This increased availability is an extension of the services offered by your practice and is provided by a range of local clinicians including GPs, nurses and health care assistants.

Please note, this is not a walk-in service – you still need to make an appointment. Appointments can be made either via your own GP practice or by calling the extended access team on 0113 221 3540 between 9am and 12pm on Saturday and Sunday.

Out of Hours

When we are closed your call will be diverted to the out of hours provider or you will hear a recorded message advising you of a number to contact the out of hours provider directly. You can also contact NHS 111 or visit www.nhs.uk

Friends and Family Test at Wetherby Surgery

75% of patients at Wetherby Surgery would recommend the service to family and friends.

This figure was taken from our NHS Friends and Family Test (FFT) which is the patient questionnaire we use to help us to understand whether our patients are happy with the services we provide. The FFT is a quick and anonymous way to give your views after receiving care or treatment across the NHS.

Meningitis B Vaccine - update

We are currently experiencing a national shortage of the Meningitis B vaccine Bexsero which means we are unable to start any new courses of the vaccine. We are however operating a waiting list for all those who wish to be vaccinated. If you would like to join our waiting list, please contact our reception team on 01937 585669.

We are not yet able to advise a timeframe of when the vaccines will be available, although we have been informed supply will remain low until the summer of this year. We will send regular updates to patients on the waiting list. If your child has already started a course of Meningitis B vaccine with us, we will honour the remainder of that course as a priority.

Please note: This shortage applies to private vaccinations only – those children who’re eligible and deemed as most at risk will still be vaccinated under the NHS programme.

Your named GP

Here at Wetherby Surgery we have nominated a named GP to be accountable for the care of each of our patients. The purpose of this is to ensure that our patients have a named individual who they know is responsible for the overall care they receive at the surgery. 

Having a named GP assigned to you DOES NOT mean that you are obliged to see that GP; you are welcome to continue to make appointments to see any of the GPs at the surgery.

If you would like to know the name of the GP who is accountable for overseeing your care, please contact the surgery and we will be more than happy to inform you.
